Please be informed that interviews for successful candidates at the 2020/2021 entrance examination have been scheduled to take place on Saturday 27th June 2020, Saturday 4th July 2020, and Saturday 11th July 2020. To this end and to supply us information about your interview date preferences and other information, kindly click on the link below to fill and submit the form provided.

Furthermore, kindly take note that candidates are required to have the following ready for the interview:
- Evidence of payment of non-refundable acceptance fee of N30, 000 (Thirty thousand naira only) transferred into Zenith bank account of Louisville Girls Secondary School; 1011234646. Kindly email this evidence email@example.com for confirmation. The subject of the email should be written as indicated below’
ACCEPTANCE FEE FOR "CANDIDATE'S FULL NAME"
After confirmation, we will send you (within 24-48 hours) an e-mail containing the medical tests form and the candidate's access code.
Please note that no access shall be granted to the medical tests form until acceptance fee payment is made & confirmed.
- Photocopy of Certificate of Birth-for submission - Issued by the National Population Commission-Federal Republic of Nigeria. In the absence of this, a photocopy of the bio-data page of her Nigeria e-passport (ECOWAS passport) can be accepted.
- Original copy of baptismal card- for sighting (for Catholics and Orthodox Church members only) Photocopy of the baptismal card- for submission
- Results of medical tests. The recommended hospitals are State or Federal Universities’ Teaching Hospitals, Catholic Hospitals, State or Federal Government Hospitals like Federal Medical Centre, General Hospital, National Hospital, etc. It is compulsory that the result carries the hospital stamp and signature of a General Practitioner or a Pediatrician.
For those for an online interview, (i)-(v) should be scanned and sent to firstname.lastname@example.org 3 days before the chosen interview date. If the medical tests cannot be done now because of the inaccessibility of medical facilities due to the COVID-19 pandemic, the school can wait until when all new students resuming are expected to submit the original copies of the medical results.
Note: Both parents are required to attend the interview with their daughter(s). Where this is inevitable, a letter of consent is expected from the other parent who will not be attending the interview.
